In order for the Garland Police Department Financial Crimes Unit to take an identity theft report, we require that you complete an identity theft packet.  The packet can be downloaded by clicking on the below listed links.  The packet is available in PDF format.  Please download and fill out the form, have the affidavit (included in packet) notarized and return it to the Garland Police Department. If you are a Garland resident, the form must be turned in to the front desk of the police department.  If you are not a resident of Garland, and do not live in the DFW area, the form can be mailed to the Garland Police Department at the below address:
